Supporting using tag <w:outlineLvl> to decide which title level tag should be used.

After reading the source code, I find that the w:outlineLvl is dropped at method map_results.
And the tag <h1>, <h2> ... are just processed by the styles. Maybe the tag should be processed by the tag: <w:outlineLvl>.
