I don't know if anyone else has seen this, but on some devices (usually Samsung) the menu button in the action bar is hidden. The user must long-press one of the built-in phone buttons, usually the task switcher. I don't know how to represent that in this site's format, or what devices are specifically affected. (It's a huge usability issue because users can't discover it.)
