Style topics as `<h4>`s and move some of them to built-in classes. Also remove some that didn't really need to be there.

When we moved to Shibuya, all the plain
.. topic::s lost their styling, so they just became weird plain text labels and looked out-of-place. This fixes all of them in ways I thought were appropriate. We now also have a dedicated.. example::for examples that includes theExample:prefix.
